CLIENT SERVICES - CASE MANAGEMENT
Case Management Services are designed to advocate for the participants rights and responsibilities by facilitating safe and timely coordination of all parties involved in a participants "team."
The Case Manager collaborates with all resources involved and develops an individualized plan of care. The Case Manager also monitors and communicates the progress of the implemented plan of care to all involved resources while acting as a resource to Team Members throughout the implementation of the plan. In addition, he/she makes revisions in the plan as suggested by Team Members, Family and the Client, on an as needed basis.
Financial Management
NOWCAP Services is dedicated to enhancing the quality of life for people with disabilities who need help in achieving a comfortable, secure, and independent lifestyle. Routine tasks such as the payment of a bill or the filing of an insurance claim form can be an overwhelming experience. NOWCAP Services mission is to eliminate this stress and confusion by providing a wide variety of services designed to keep its participants' financial affairs in order and to minimize their vulnerability to financial abuse.
In Home Services
In-Home Care is one of the least restrictive care options. If an individual is safely able to continue living at home, arrangements may be made to have a caregiver go to the client's home. The caregiver may provide assistance with housekeeping chores that the client is no longer able to do, such as cleaning, grocery shopping, and cooking. The caregiver may also help the individual with personal care tasks, such as bathing, toileting, dressing, and feeding. In addition to the skills taught by the caregiver, recreation and leisure opportunities may be provided.
